My mom suffered a stroke in August 2016. Since then she has been in a nursing home. I'm unable to find any photo ID and her SSI card. We live in PA. I spoke with the social security office and I have to download a SS5 form and have my mom sign it. What kind of ID do they accept? From what I understand they don't accept copies and want originals. Do I go to the nearest SSI office?
To replace license, you will need those documents, and others depending on your state. Contact your DMV. I was able to take mthr down to the office with her bc and ml along with what her ssn # was without the card. They were more than glad to issue a new ID card and whispered to me, is she turning in her license. Of course I said yes! They were so happy. Mthr had hypoxia so she was really out of it, and it was a smooth operation.
