Contact the local department of transportation, because he is "blind" he qualifies for a free companion to ride the transportation with him. Depending on your area this can be a b7s, a van, a non-emergency medical car or even a taxi. Because he is low income he will most likely qualify for free rides.

You have to have an application sent via mail and then you fill it out and return the same way, so it takes several weeks to get set up. They do door to door transit.

He is legally blind and a poor historian. He relys on me to answer all his questions as he doesn't hear well either. I help pay his bill's, etc. He has multiple physical issues and none can wait. As he is extremely low income it's difficult to ask for gas money.

I shouldn't worry so much about taking him to his doctors' appointments just at the moment. Is there anything that really can't wait?

He can pay for the gas and compensation you for your time. But no, there’s no financial assistance if all you provide is transportation to doctors appointments.
