Unobtrusively, in case she wanders?

There are luggage tags, Air tags that you can attach.
If she has a cell phone you can track that.
Some watches also have the ability to be tracked.
Do notify the police in your area that there is a vulnerable senior that may wander. Some have profiles you can fill out so if the person does go missing they can get the info quickly.
Some communities also have programs that will track vulnerable people. (Autism, dementia. In general they do have to have 24/7 supervision so the system is not "babysitting)
You also might want to look into an Adult Day Program so that for at least a good portion of 2 or 3 days a week they are supervised giving you a break.

She has no internet/wi-fi or anything like that nearby - don't most options require that?

You can easily attach a lost luggage finder tag; would require drilling a hole through and attaching it. As to how you explain for the elder herself, it can be as simple as "This is a new-fangled "cane finder" alert button in case you lose or someone takes your cane. Truth is they love their "stuff" protected and get quite paranoid about losing their things. Might work.

However, if wandering is now a problem there are bigger issues afoot and I am afraid it is a situation of no longer being able to be alone; this may require placement. There comes a time when safety demands that there are several shifts with several folks on each to keep an elder with dementia somewhat safe. I am certain you recognize that.
