Your loved one may not require home care or assisted living services at this time. However, continue to monitor their condition for changes and consider occasional in-home care services for help as needed.

In order to have answers from this Forum you will have to participate in it. You will need to answer questions. My question is "Who is POA for your ex of Medical POA?"
Do know that Licensed Social Workers in private counseling practice are listed online for your area. You have asked this question several times, so you might consider paying for an hour of time to collect information on your options with a visit to such a professional.
You might also consider a visit to an elder law attorney. A POA is reimbursed out of the principal's funds for this expert advice.
You have asked a similar question 2x before with answers. You have not responded to questions asked.
One question asked was do you have Power of Attorney and if so is it in effect? Has your ex been found incompetent?
What you need to do is what Geaton has suggested, go to your County Social Service Office and speak to a Social Worker there. That person will help you thru the Medicaid application and tell u what you need to do.
If your ex is hospitalized, tell the SW there you can no longer take care of him and he needs to be placed. To release him to home would be an "unsafe discharge". You refuse to pick him up. If you have no POA, tell the SW that the State will need to take over his care. Walk away, don't answer ur phone.
Everything I have written has been written to u before. There is nothing we personally can do for you. You need to do this yourself.
What do you mean “ we don’t do that “? All the Dr.’s office needs to do is send a referral to the social work department that is connected with his network. It’s very simple. There is a social work department at every major hospital in your area. His comment doesn’t make sense. But if that’s really the case, your next step would be contacting your local Agency on Aging, or if you don’t get help from them, contact your state’s elder protective services.
You contact Social Services for the county where your ex resides (usually the Dept of Health and Human Resources).
If you aren't the PoA or legal guardian for your ex, you may have to report them to APS as a vulnerable adult if your ex doesn't (or can't) request the social worker themselves.
How old is your ex?
What is the main problem that you think requires the attention of a social worker?
Please know that if your ex doesn't have a PoA and is having cognitive problems, the county does have the power to acquire guardianship for him/her. In this scenario they will begin managing all their affairs and find an appropriate facility for him/her and then your ex will have all their needs met.
