My brother is 65 and I am 60 we own a house in Joint. Is there legal document we could sign if giving the other full ownership should one of us get sick or go into a nursing home?

You need to consult a lawyer familiar with Medicaid in your state. It depends on how the deed is written - if it is joint with right of survivorship, tenants in common etc. If one of you goes into a nursing home and requires Medicaid, the half of the house that is owned by that person will have a lien put on it. Since there is a 5 year lookback, anything you do with property transfers will show up in the state's database. A lawyer can help you figure out the best way to handle it. It's not a do-it-yourself project if you are not familiar with real estate law or Medicaid.
