Find Care & Housing
I'm no expert and I know that Medicaid varies by state, but I know someone in my state who is disabled and she gets Medicare (she's under age 65) AND Medicaid. Her Medicare pays first and then Medicaid kicks in, so she has no out of pocket medical expenses.

Depending on what state you are in, I would also check on something that is called Extra Help that the state may offer. It covers out of pocket costs for Medication that aren't covered elsewhere.

The rules vary from state to state. In Ohio, each county has an office (it's called Job & Family Services). This is the place (or the equivalent where you are from) where you should be able to get information that is relevant to your situation.
My personal experience is that I was able to get my FIL on Medicaid even though he is a Medicare beneficiary. He is age 80 so his situation is different than yours. We went through a spend down of the few assets he had so he would qualify. In his case, Medicare pays first for his medical and prescriptions, Medicaid picks up the rest. He also gets Extra Help and doesn't pay premium for his Medicare Part A or his Medicare prescription drug plan. The whole process can be daunting, I would advise being VERY VERY nice to any assigned caseworker and establish a good rapport with him/her. :) I got an angel who helped me at every turn.
