Yes, a doctor can charge to fill out order forms for you. No insurance will pay for this. Like said, if you go directly to a durable equipment store, they will probably fill out paperwork because they will be reimbused by Medicare for the sale.

I think each doctors office handles paperwork differently.

Contact the office, there is usually a separate office or number for billing and or insurance questions. those are the people you want to talk to.

It is very possible that the Medical Supply store you are using can contact the doctors office and have the authorization sent to them it may not be at a cost to you.

What do you mean "charge me for filling out".
Do you mean can your doctor's office charge for turning in forms requesting medical equipment? I honestly don't know. I don't know if such a service is covered under medicare as it is not a medical procedure. I know they charge for making chart copies and so on. This is question for your doctors office.
