Our concern is that she has a full resuscitation order in place.  She is in renal failure, but refuses dialysis. I don't believe her thinking is sound enough to understand the conflicting orders she has in place. She has an advanced directive, but her current POLST would require paramedics to resuscitate if she went down in the Assisted Living facility. The caregivers and several nurses have advised we get a Medical POA.

Even if you have a medical POA you can't override her advanced directives (although you should probably have one regardless). See if you can find a hospice nurse who will sit down and discuss this with her.
