She has macular vision problems that makes it hard for her to see even the tv clearly. This coupled with some OCD's issues makes her anxious and afraid. She presents Mid mornings and up to her afternoon nap time she is like " the mother I always knew". After her nap she is quite disoriented and needs more cues and directions with ADL's and is unclear of time of day or even what day it is. I want this move to be where she feel safe as well as with peers that are at her stage of dementia. Thanks for any help you can give me.

You could check local assisted living centers that have memory units. Her other health issues may make her a candidate for a nursing home, however. I'd check both to see what is best. Also, if she needs to be on Medicaid (doesn't have many assets), a nursing home is the best choice.
