She has memory problems, needs transport to and from medical

Usually a POA is responsible for this. Does she not have one?
Does the Assited Living not supply transportation for appts and shopping?

Has Mom been paying for her AL privately and now running out of money? Does her AL except Medicaid? Are you not close by? Have you talked to the Administrator?

Who hold's Moms POA? This should be the person who handles Moms paperwork. Doesn't the AL supply transportation?

So Mom is on Medicaid? Is Medicaid paying for her AL and does she have Medicaid for medical too? Medicaid usually provides a transportation service but you must sign up. Counties have Senior busing usually thru the Office of Aging. The yearly renewal form is pretty easy. You just supply updated information. Like her bank statement. Probably need to show that any pension and SS is going to her care if Medicaid is paying for her AL. If you are not nearby, ask if Medicaid can have paperwork sent to you.

Medicare paperwork is just a statement. She sees a Medicare/Medicaid provider, they bill medicare, medicare pays 80% of what they allow, they send it to Medicaid who pays in full the 20% left. My nephew says he has never gotten a statement from Medicaid or a bill from a doctor. For me this is cut and dry too. No reason to have anyone oversee it.

I also think if the AL allows Medicaid to pay for rent, etc for a resident, someone there should be able to help with Medicaid forms. Also, if they don't supply transportation, who does. I worked for a Visiting Nurse Assoc. and I had to know every resource available to our clients and Township residents. I updated every year any county resources.

Her County Office of Aging maybe of some help.
